When there is an issue such as a broken component or lost item, users may submit the issue via OperationsPath Shared Services Module.


  1. To submit an issue, navigate to OperationsPath Shared Services, select the category of the issue, and click Issue.
  2. Select New located in the top table bar.
  3. Fill in the appropriate information into the form. Press Learn How to Submit a ___ Issue for a quick tour of the form inputs.
